Thank you for taking the Disciple Maker Index Survey! This survey is part of the Diocese of Columbus's "Real Presence, Real Future" strategic plan, and your feedback will help St. Matthew the Apostle plan for the future, strenghten our community, and be better disciples to the world. The easiest way to take the survey is online. It should take about 10-15 minutes to complete. However, if you prefer to fill out a paper copy of the survey, you can print one using the middle button below, or you can pick one up in the Gathering Space or in the parish office. Please return completed paper surveys to the parish office or in the collection basket in an envelope marked "survey." The survey is open from February 17 (Ash Wednesday) to March 21. All parishioners, age 16 and over, are invited to take the survey.
