- This form is for school and parish staff/ministry heads who have already scheduled events on the parish or school calendar. If you have not scheduled your event, please contact Hillarie Michaels at hmichaels@stmatthew.net or (614) 471-0212.
  
  Note: School events that are happening in the school building after school hours need to be on both the parish and the school calendars, as the parish also uses space in the school building outside of school hours.
  
  Once submitted, this form will be sent to Facilities Superintendent, George Harvey for review.
